Your First Visit
When you reach out to schedule, our office manager, Denise, will give you a call to learn a little about you and get your appointment set up. You'll also receive a confirmation text, along with a link to complete your new patient forms digitally before you arrive.
When you walk through our doors, you'll be warmly greeted and comfortably settled in our waiting area, though most patients are back with our team within just a few minutes! A member of our hygiene staff will bring you to your operatory, take any necessary images, and take the time to hear what brought you in today before getting started.
Patient Forms
To help make your first visit as smooth as possible, we offer downloadable new patient forms right here on our website. Completing them ahead of time means less time spent on paperwork when you arrive and more time focused on your care. Simply download, fill out, and bring your completed forms with you to your appointment, and let us know if you need any assistance.

If we are not a preferred provider for your insurance, we are still happy to work with you to maximize your benefits by processing and filing insurance claims.
